Reactively you should probably do the following: First and foremost get a copy of the .exe being created on the file share and get that off to your AV vendor. Reload to refresh your session. I suspect its something hidden, as I ran a boot-time scan twice over, and it didn't find the source, though it did find other crap like rootkits and Trojans.
